I am reminded of a patient who shared her thoughts with me when i told her that she was in control of the decision regarding heart transplantation as a treatment for her advanced form of heart failure. Though a number of improvements in artificial hearts have been madethere are now small, internally implanted models that dont require huge external machinesthey arent, as of yet, the panacea people initially hoped they would be. Even though youre asymptomatic, you can determine if youre at high risk for heart failure by asking yourself the following questions: have you ever had a heart attack? Have you ever had any infection of the heart or a previous inflammation around the heart? Do you have a heart murmur? Are you overweight? Do you have high blood pressure? Are you a heavy drinker, smoker, or drug abuser? Do you have diabetes? Do you have a family history of heart failure? (prevailing medical opinion used to be that there is no correlation between heart failure and family history; in the face of mounting evidence to the contrary, that opinion is now changing. Between the two layers of the pericardium is a thin space filled with a watery fluid that helps prevent these layers from rubbing against each other when the heart beats. The inner surfaces of the hearts chambers are lined with a thin sheet of shiny, white tissue known as the endocardium. In caring for patients awaiting heart transplantation, we have observed that in those who wait prolonged periods of time for a donor to become available (and the wait nowadays often is months to years, and much of the time is spent in a hospital on support medicines and devices!
